The movie is directed by Ramesh Varma, who also co-wrote the screenplay with Ram Kumar, the director of the original Tamil version. The Story of Gumnaam Hai Koi

The movie begins with a mysterious murder of a teenage girl, who is found hanging from a tree with her face mutilated. The police are clueless about the motive and the identity of the killer. Meanwhile, Arun Kumar, who had quit his filmmaking career due to a personal tragedy, joins the police force as a sub-inspector. He is assigned to the murder case and starts his investigation.

He soon finds out that the girl was killed by a serial killer who follows a pattern of killing girls who resemble Mary Fernandez, a character from a novel called The Psycho. The novel is based on the life of Christopher, a real-life serial killer who was executed for his crimes. Arun also learns that the killer leaves behind clues in the form of origami figures and music boxes that play the song Gumnaam Hai Koi, which was Christopher's favorite song.

Arun tracks down the author of the novel, Doss, who reveals that Christopher had a twin brother named Albert, who was also a psychopath and had escaped from a mental asylum. Arun suspects that Albert is the killer and tries to find him. However, he faces many obstacles and dangers along the way, such as corrupt cops, false leads, and traps set by the killer. He also falls in love with Krishnaveni, who works as a teacher in his niece's school.

Arun eventually discovers that Albert is none other than Saravanan, his senior officer and mentor in the police department. Saravanan had been killing girls to avenge his brother's death and to fulfill his brother's unfinished mission. He also kidnaps Arun's niece, Anjali, who looks like Mary Fernandez, and plans to kill her on Christopher's birthday. Arun manages to find Saravanan's hideout and confronts him in a climactic showdown. He rescues Anjali and kills Saravanan, ending his reign of terror.
